雷维模，四川武胜县人，四川音乐学院扬琴专业毕业。西南大学育才学院音乐学院教授，教育部高等学校高职高专表演艺术类专业指导委员会四川省分委员会副主任。系中国音乐家协会会员、中国音乐著作权协会会员、中国社会音乐研究会理事、四川省社会音乐研究会副会长兼秘书长。曾任四川省群众艺术馆抒情歌曲编辑部主任，四川省文化馆理论信息部主任，四川省音协第四、五届理事，育才学院音乐学院副院长等。
自20世纪70年代以来，在全国数十家报、刊、台发表歌曲400余首、音乐文论近百篇，获"群星奖"、"五个一工程"奖等全国及省部级奖励若干。《五十六个民族一个中华》《中国，欢乐的海》、《丝路驼铃》、《芦沟晓月》、《大山里长大的吊脚楼》、《在小河淌水的地方》等几十首作品收入《中华百年歌典》等多家出版社的20多种歌曲专集，有的还收入大、中、小学音乐教材。2001～2010年十年间，在成都、重庆成功举办三次个人作品音乐会。
出版有《丰收的中国》、《中国的节日》、《滑溜溜的山路溜哟溜》、《雷维模声乐作品选》等个人作品集。其它音乐著述有高等学校音乐专业教材《中国民族音乐概要》及《实用简谱读法》等。此外，还为多家出版社选编《20世纪中国名歌》、《20世纪中国影视剧歌曲》、《20世纪外国名歌》、《红烛颂•绿叶情》、《祖国颂》、《放歌90年》以及《抒情歌曲选》、《抒情歌曲精选》、《九百九十九朵玫瑰》、《中老年喜爱的歌》等各类歌曲专集50多本。
